Essential Functions


Delivery of Patient Care



  • Assumes responsibility for the coordination and delivery of patient/caregiver care.

  • Conducts nursing assessments to assist in the overall management of patient/caregiver needs.

  • When appropriate, develops, implements, evaluates, and updates the plan of care to ensure care is uninterrupted and that goals are achieved.

  • Facilitates patient and caregiver independence to the extent possible.

  • When providing hospice care, appropriately utilizes VIA Health Partners patient‑related protocols, standing orders and guidelines.

  • Assumes responsibility for conducting effective patient and caregiver education.

  • Encourages and allows time for questions and consultation.

  • Provides education to patients and families as needed.


Interdisciplinary Group



  • Attends, participates, and may facilitate interdisciplinary group conferences (IDG) and special care conferences.

  • Assumes responsibility for establishing and maintaining effective working relationships with team members.

  • Identifies specific patient/caregiver needs and makes referrals to appropriate team members.


Documentation



  • Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.


Professional and Personal Accountability



  • Demonstrates competency in knowledge and skills.

  • Ensures that VIA Health Partners professional reputation is maintained and projected.

  • Assumes responsibility for professional development and staying abreast of current trends in the healthcare field, incorporating new information and methods into practice.

  • Maintains current professional licensure, CPR certification and CEU requirements.

  • Maintains productive and professional relations with external agencies and healthcare providers.

  • Exhibits time‑management skills and can plan the workday while remaining able to respond appropriately to crisis.


Other Duties



  • May perform other duties as required, e.g., cleaning patient rooms according to patient‑room cleaning procedures, ensuring all facility areas are clean, stocked, and that protective equipment is used when working with chemicals.


Qualifications



  • Diploma or associate degree in nursing (Bachelor’s degree preferred).

  • Current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN).

  • Current CPR required; CHPN preferred.

  • Minimum one year of related experience for inpatient units (IPU) and two years for other nursing positions. Experience in direct care of patients with life‑limiting illnesses required; hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical‑surgical experience acceptable.

  • Computer proficiency required.
