Job Description

Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a part‑time hospice Registered Nurse to work 2 PM – 10 PM in the Morris County area, including weekends and on‑call as required by department. The role works with adult and geriatric patients of all backgrounds who require hospice care at hospital sites or in patients’ homes as needed. The RN will manage a heavy caseload (four patients a day) and provide individualized time and attention to meet each patient’s needs. Entry level compensation starts at $43.00 per hour and increases based on experience; mileage reimbursement at $0.70 per mile is included. Internal referral bonus: $4,000.

Reports to: Assistant Manager

Duties & Responsibilities

  • Accurately assess patient’s appropriateness for hospice care, learning needs, and progress toward goals.
  • Develop the plan of care for the patient and family.
  • Maintain visit productivity standards.
  • Demonstrate knowledge of insurance reimbursement.
  • Perform laptop transfer of data twice daily.
  • Complete admission assessment and transfers within 24 hours.
  • Complete re‑visit documentation and transfers.
  • Evaluate teaching plan and revise as necessary.
  • Document patient education tools used.
  • Develop, implement, and evaluate plan of care based on evidence‑based knowledge.
  • Revise care plan as indicated.
  • Prioritize care needs.
  • Demonstrate knowledge of hospice regulations and insurance reimbursement guidelines.
  • Show evidence of interdisciplinary communication.
  • Perform HHA supervisions per standards.
  • Ensure appropriate utilization of services.
  • Follow scheduling process.
  • Communicate progress toward goals with IDT.
  • Attend and participate in IDT meetings every 2 weeks.
  • Evaluate the plan of care and revise as needed.
  • Follow infection control measures and report infections.
  • Demonstrate knowledge of pharmacology and safe medication management.
  • Routinely reconcile medications.
  • Maintain current medication orders.
  • Participate in PI meetings and activities/committees as requested.
  • Accept other assignments as requested.
  • Prioritize schedule to accommodate patients’ needs.
  • Document case‑management activities.
  • Review health records and gather data to support PI activities.
  • Attend UR meetings as requested.
  • Provide orientation/mentorship to new employees.
  • Cross‑train to learn new skills and demonstrate a willingness to teach others.
  • Assist with growing agency business by providing orientation to new employees.

Required

  • Current RN license.
  • Associated Degree in Nursing.
  • Valid and unrestricted driver’s license with a clean driving record.
  • BLS certification.

Preferred

  • Bachelor’s degree in nursing.
  • All ASN must complete their BSN studies within five years of their start date.
  • Master’s degree in nursing or related field.
  • 1+ year of experience in nursing or a relevant area.
  • ACLS certification.
