Key Responsibilities
- Provide management and delivery of individualized hospice patient care plans
- Conduct comprehensive assessments and ongoing evaluations of patient needs
- Consistently meet critical deadlines for charting and documentation
- Collaborate with interdisciplinary hospice team members to ensure coordinated care
- Educate patients and families on care plans, symptom management, and available resources
- Self-manage time, schedule, and daily patient visits
- Maintain compliance with all regulatory, clinical, and organizational standards
Schedule & Shift Details
Full‑time weekday schedule
On‑call rotation: Every 3rd week
Occasional local travel to patient homes and facilities
Required Qualifications
- Associate Degree or higher
- Valid Registered Nurse (RN) license in Pennsylvania (PA)
- 2+ years of Hospice experience
- 1+ years of experience in any of the following: Electronic Medical Record (EMR), Other EMR systems, TruChart EHR
- Strong ability to self‑manage time, schedule, and documentation
- Compassionate approach to end‑of‑life care
- Excellent communication and patient advocacy skills
