What You’ll Do
- Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
- Identify the patient's/family's physical, psychosocial, and environmental needs, reassessing no less than every 15 days.
- Initiate and coordinate the plan of care while maintaining accurate clinical notes.
- Document problems, goals, interventions, and patient/family responses to hospice care.
- Collaborate with the patient/family, attending physician, and other IDG members in daily care.
- Function in any home situation regardless of the client's age, race, creed, color, sex, disability, or financial condition.
Requirements
- Education: Current unrestricted RN licensure in the state of practice
- Experience: 1+ years of clinical experience preferred
- Licenses & Certifications: Current driver’s license and valid vehicle insurance required; current CPR certification or ability to complete within 90 days of hire preferred
