Role:
Hospice Nurse providing end-of-life care in patients' homes and facilities.
Responsibilities
- Assess and manage patient symptoms, administer medications, and coordinate interdisciplinary care.
- Support patients and families through physical, emotional, and spiritual needs.
- Collaborate with physicians, social workers, chaplains, and therapists to develop and implement care plans.
- Contribute to patient and family education and document care in the EMR.
- Maintain clinical excellence, respect, and cultural humility in all interactions.
