Role Description
The Hospice Nurse provides direct patient care, including comprehensive assessments, symptom management, medication administration, and coordination of individualized care plans. The Hospice Nurse collaborates closely with an interdisciplinary team to deliver palliative and hospice services, offers education and emotional support to patients and families, and advocates for patient comfort and dignity. Daily responsibilities include monitoring patient status, documenting care in accordance with clinical and regulatory standards, communicating with physicians and care partners, and participating in care conferences and quality initiatives.
Qualifications
- Current Registered Nurse (RN) license in the state of Wisconsin.
- Excellent communication, active listening, and interpersonal skills with patients, families, and colleagues.
- Ability to work collaboratively in an interdisciplinary team and adapt to changing patient needs.
- Strong organizational and documentation skills, with attention to clinical and regulatory requirements.
