Position Overview
We are seeking an experienced Hospice Liaison RN to support hospice referrals and patient transitions across hospitals and skilled nursing facilities in Northern Wisconsin. This role focuses on hospice assessments, patient education, and coordination of end-of-life care services.
Coverage Area
Assignments may include:
- Green Bay
- Oshkosh
- Manitowoc
- Grafton
Travel will vary by assignment and hospital coverage needs.
Key Responsibilities
- Review and manage hospice referrals from hospitals and SNFs
- Assess patients transitioning to hospice care
- Recommend appropriate hospice services and levels of care
- Complete hospice admissions within hospital settings
- Educate patients, families, and providers on hospice philosophy and benefits
- Collaborate with physicians and interdisciplinary care teams
Adult patients with:
- Cancer
- End-stage and terminal illnesses
Requirements
- Minimum 2 years of hospice experience preferred
- First-time travelers accepted
- BLS Certification required
- Valid Driver’s License, Auto Insurance & reliable vehicle required
- EPIC experience required
Skills Required
- End-of-life symptom management
- Hospice medication knowledge
- Strong communication and provider collaboration
- Understanding of hospice levels of care:
- Routine
- Respite Care
- Ability to educate patients and families on hospice services
- EMR: EPIC Hyperspace & EPIC Remote Client
- Laptop and phone provided
- Support from:
- Hospice Medical Directors
Orientation & Additional Information
- NEO Orientation provided
- 40 hours of onsite training with staff
- Business casual or professional scrubs allowed
