Elara Caring is seeking a Hospice Nurse Practitioner in New Braunfels, TX for part-time, PRN coverage. You will perform hospice recertifications, admissions, and care planning; assess and document patient needs; and coordinate with physicians and the interdisciplinary team to manage symptoms in the home setting.
