Responsibilities

• Proactive, collaborative team member in a hospice care environment
• Conduct comprehensive adult/geriatric assessments of hospice patients and evaluate and manage plan of care
• Commitment to caring for patients and partners
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting

Qualifications

• Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice
• Board‑certified by AANP or ANCC in adult, family practice, geriatric, or adult‑gerontology primary care

Licensure, Certification, and Educational Requirements

• Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
