NURSE PRACTITIONER

We are seeking a dedicated Nurse Practitioner to provide compassionate hospice care in patients’ homes or care facilities.

Responsibilities
- Proactive, collaborative team member in a hospice care environment
- Conduct comprehensive adult/geriatric assessments of hospice patients and evaluate and manage plan of care
- Commitment to caring for patients and partners
- Complete all documentation and records in a timely and accurate manner
- Follow established policies and procedures as related to the scope of practice
- Maintain continuity of patient care in a hospice setting
Licensure, Certification, and Educational Requirements
- Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice
- Board‑certified by AANP or ANCC in adult, family practice, geriatric, or adult‑gerontology primary care
