Key Responsibilities

  • Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
  • Conduct comprehensive nursing assessments and implement individualized plans of care
  • Deliver skilled nursing services focused on comfort dignity and quality of life
  • Administer and monitor pain and symptom management interventions
  • Educate patients and families on disease progression care plans and symptom control
  • Document patient care accurately and timely within electronic medical records EMRCollaborate with the interdisciplinary team to ensure coordinated holistic care
  • Evaluate patient responses and adjust care plans as needed

Required Qualifications

  • Graduate of an accredited PracticalVocational Nursing LPNLVN program
  • Current LPNLVN license in the state of Ohio
  • Minimum 3 years of nursing experience hospice home health med surg or community health preferred
  • Current BLS certification
  • Strong clinical communication and organizational skills
  • Valid drivers license reliable transportation and proof of insurance
  • Understanding of hospice philosophy and end of life care

Preferred Qualifications

  • Experience in hospice or palliative care
  • Familiarity with home based care and EMR systems
