As a Hospice Liaison, you will:
- Build & Nurture Referral Relationships - Develop and maintain strong partnerships with physicians, discharge planners, skilled nursing facilities, and community organizations to support timely and appropriate hospice referrals.
- Educate & Advocate - Promote hospice philosophy through compassionate education to referral sources, community groups, and the general public. Serve as a knowledgeable guide for families considering end-of-life care options.
- Support Admissions & Transitions - Collaborate with clinical teams and intake staff to assess patient eligibility and ensure a smooth, dignified transition into hospice services.
- Lead Community Engagement - Represent Vivie Hospice at community events, professional gatherings, and health fairs. Share resources and build brand awareness.
- Partner with marketing to refine outreach tactics and materials that resonate with the communities we serve.
- Analyze & Report - Monitor referral activity and trends. Provide regular reports and feedback to leadership to support growth and quality outcomes.
- Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
- Clinical background (RN, LPN, or social work) is preferred but not required.
- 2+ years of experience in hospice, healthcare outreach, or a related field.
- Strong understanding of hospice and palliative care principles.
- Ability to manage time independently, prioritize multiple responsibilities, and respond to evolving needs.
- Valid driver's license and reliable transportation.
- Experience with EMR and CRM systems.
- Familiarity with intake workflows and patient admissions processes.
- Ability to pass state mandated background checks.
- Physical capability to perform all essential job functions.
- Ability to read, write, and speak English to ensure effective communication with team members, clients, and visitors.
