Hospice Field Staff RN/Case Manager

Hospice of Southern Illinois is a not‑for‑profit, free‑standing, community‑based hospice offering comprehensive end‑of‑life care in Southern Illinois and the St. Louis Metro‑East area. Currently hiring for a Hospice Field Staff RN/Case Manager based out of our Belleville, IL office.

Position Summary

  • The Staff Registered Nurse is responsible for assessing patient/family/caregiver needs, planning and implementing the Plan of Care, and evaluating effectiveness. As a member of the interdisciplinary team (IDT), the RN/Case Manager oversees patient care throughout hospice.
  • Pay: $39.21 hourly. Full‑time, non‑exempt. 40 hours per week.
  • Schedule: Monday‑Friday 8:30 AM–5:00 PM with additional rotational on‑call schedule (evenings, weekends, holidays).
  • Location: Belleville Patient Services Office. Primary patient visits within the Belleville service territory (St. Clair, Monroe, Randolph, Washington). Extensive travel may be required.
  • Valid driver’s license, current auto insurance, and reliable transportation required. Company reimburses $0.70 per mile for personal vehicle use.

Position Requirements

  • Education
    • Graduate of a state‑approved nursing school.
    • Associate Degree or Diploma in Nursing required.
  • Experience
    • One year in hospice settings preferred.
  • Other Qualifications
    • Current Illinois RN license.
    • Hospice and palliative care certification preferred.
    • CPR certification preferred.
    • Strong leadership, clinical assessment, organizational, and interpersonal skills.
    • Knowledge of Medicare/Medicaid, hospice regulations, and third‑party payer expectations.
    • Reliable transportation.

Physical Requirements

Perform repetitive tasks, including bathing, transferring, lifting (up to 100 lb occasionally, 50 lb frequently, 20 lb constantly). Requires bending at the knee and squat‑bend. Requires clear vision >20 inches and <20 ft, hearing alarms, phone, and recorder.

Working Conditions

Approximately 20% of time in an air‑conditioned facility, 80% in patient homes, vehicle, or other environments. Exposure to humidity, noise, odors, body fluids, allergens, cleaning chemicals, communicable diseases, and traffic is possible.

Essential Functions

  • Maintain a caseload of patients and oversee care coordination.
  • Provide direct patient care, including on‑call after‑hours services.
  • Demonstrate required RN clinical skills and participate in annual skills day.
  • Conduct physical assessments, medication reviews, symptom and pain evaluations on each visit.
  • Enter medication orders accurately and obtain physician orders as needed.
  • Communicate findings to hospice physician and obtain orders for care management.
  • Review and update the Plan of Care (POC) at each visit.
  • Initiate, supervise, and update Hospice Aide plans per Medicare regulation.
  • Provide patient, family, and caregiver education on disease progression and pain/symptom management.
  • Identify psychosocial, emotional, and spiritual needs; collaborate with IDT.
  • Document assessments systematically per Policies and Standards.
  • Oversee Licensed Practical Nurse activity.
  • Triage on‑call calls and apply critical thinking.
  • Act promptly during emergencies or crisis events.
  • Offer emotional, spiritual, psychosocial support and establish therapeutic relationships.
  • Present updates at IDT meetings (every 15 days).
  • Work cooperatively within a team environment.
  • Embrace change, manage stress, maintain a positive attitude.
  • Build relationships with internal and external customers; promote referrals.
  • Participate in QAPI activities; maintain compliance with performance improvement measures.
  • Organize tasks efficiently with minimal supervision; self‑starter initiative.
  • Contribute ideas to improve work processes.
  • Incorporate Hospice of Southern Illinois core values.
  • Manage paid time off, submit online requests.
  • Maintain accurate timesheets per policy.
  • Other duties as assigned.
