Hospice Field RN/Case Manager: Impactful Home Care

Hospice of Southern Illinois is seeking a Field Staff RN/Case Manager based at the Belleville Patient Services Office, serving the Belleville area and surrounding counties. The RN/Case Manager assesses patient and family needs, plans and coordinates care, and manages the Plan of Care in collaboration with the IDT.

This full‑time role requires a state‑approved nursing license, one year hospice experience preferred, and on‑call duties including evenings, weekends, and holidays.
