Hospice Clinical LiaisonThe Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. This role develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners. The ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process.Responsibilities:Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partnersPlan and coordinate marketing events, educational sessions, and community luncheonsEducate healthcare stakeholders on hospice and palliative care servicesSupport patients, families, and referral partners throughout the referral and admission processProvide hospice informational / goals of care conversations with patients and familiesFacilitate initial patient consent processAttend weekly meetings to report successes, share competitor insights, and identify growth opportunitiesMeet or exceed performance expectations set by the Director of Business Development and Executive Director leadershipQualifications and Skills:LVN, RN, or MSW required1–2 years of healthcare marketing/sales experience preferredMinimum of 2 years hospice experience highly preferredStrong familiarity with local referral partners including:HospitalsPhysiciansBoard & Care (B&C)Assisted Living Facilities (ALF)Residential Care Facilities for the Elderly (RCFE)Skilled Nursing Facilities (SNF)Excellent written and verbal communication skillsSelf-directed with the ability to work independentlyDetermined, collaborative, and strong work ethicConsistent, persistent, patient, and compassionate interpersonal approachValid driver’s license, clean driving record, and current auto insurance requiredCompensation & Benefits:Salaried position with performance-based bonus potentialStarting salary at $80,000+, based on experienceMedical, Dental, Vision, and Chiropractic insurance401(k) with company matching
