Responsibilities

  • Provide skilled nursing care to hospice patients in homes, facilities, assisted living communities, and other care settings in accordance with the plan of care
  • Participate in an on‑call rotation
  • Perform routine visits, admissions, recertifications, PRN visits, and death visits as needed
  • Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
  • Administer medications and treatments as ordered while ensuring patient comfort and safety
  • Monitor changes in condition and communicate updates to physicians and team members promptly
  • Educate patients and families regarding hospice services, symptom management, coping strategies, and end‑of‑life care options
  • Support the emotional and spiritual needs of patients and families during this critical time
  • Complete all clinical documentation accurately and within 24 hours of each visit
  • Participate in biweekly IDG (Interdisciplinary Group) meetings
  • Maintain compliance with Medicare, state regulations, and company policies
  • Provide after‑hours support during assigned on‑call rotation

Qualifications

  • Current and active Texas Registered Nurse license in good standing
  • Minimum 2 years of nursing experience required
  • Minimum 1 year of Hospice Nursing experience required
  • Strong clinical assessment, time management, and critical thinking skills
  • Excellent communication and interpersonal skills
  • Ability to work independently while collaborating effectively with a team
  • Compassionate, patient‑centered approach to care
  • Current BLS/CPR certification required
  • Reliable transportation, valid driver’s license, and current auto insurance required
  • Experience with EMR systems preferred (Axxess experience a plus)
