- Manages a nursing caseload as assigned by supervisor
- Makes an assessment of the patient and family's needs to develop the nursing care plan
- Initiates the admission and plan of care
- Confers with physicians and other hospice team members to develop the interdisciplinary Hospice plan of care
- Evaluates ongoing the patient and family’s psychosocial, spiritual, and physical needs
- Directs the care given by Home Health Aides, respite aides and volunteers
- Rotates through weekend, holiday, and 24-hour call as assigned by supervisor
Requirements
- Graduate of an accredited school of nursing
- 2 years experience as a RN
- Preferred - Hospice or Home Health experience
- Required within 30 days of hire: National Provider BLS - American Heart Association
- Required upon hire: California Registered Nurse License
