Based in Brooklyn at 1-2 Hospitals!
- Evaluates patients referred for Hospice and /or Palliative Care services.
- Manages and facilitates the transition of patients in hospice or palliative care in accordance with applicable law, regulations and agency policies and procedures.
- Provides clinical support, leadership and oversight for growth and development of community and facility based hospice programs
- Duties include planning, delivering customer service, development for service
- area expansion, coordination of internal and external clinical marketing efforts, community outreach, education, and clinical service/support for hospice patient/family direct care
- Education: Graduate from an accredited School of Nursing; BSN preferred.
- Experience A minimum of two years’ experience in either Community Health. Hospice or Oncology nursing preferred.
- Business experience/oriented
