Responsibilities
- Participate in the decision-making process in relation to admission to hospice at home or in the inpatient setting
- Participate as a member of the Interdisciplinary Team in the development of individualized care plans
- Certify and re-certify patients eligible for hospice care under the Medicare Hospice Benefit
- Consult with attending physicians regarding pain and symptom management
- Coordinate efforts with the attending physician to provide continuity of care
- Develop and monitor the medical component of the hospice program
- Conduct home visits in person and virtually as a consulting physician when appropriate
- Act as a hospice liaison and advocate with national, state, and local healthcare professionals and the community
Shift & Schedule
- Forty-eight (48) on call weeks per year: Monday 7am through Friday 7pm
- Mandatory in person attendance of 1 Inter Disciplinary Team Meeting (IDT) per week
- Four (4) weekends per year: Friday 7pm through Monday 7am
