Role Overview:
Provides specialized nursing care to hospice patients in their homes or facilities, emphasizing pain management, symptom control, and holistic support. Acts as a case manager when assigned, coordinating all aspects of patient care.
Key Responsibilities:
- Assess patient preferences, needs, and barriers, including cultural and emotional factors.
- Develop, implement, and document personalized care plans in collaboration with the interprofessional team, patients, and families.
- Promote respectful, equitable, and empathetic interactions with diverse populations, supporting emotional, spiritual, and cultural needs.
- Monitor patient condition changes, communicate updates to families and team, and adjust care plans accordingly.
- Ensure evidence-based practices and quality outcomes through continuous evaluation of nursing care.
Qualifications & Skills:
- RN license with a valid driver’s license.
- Associate’s degree in nursing (bachelor’s preferred).
- Less than two years of experience required; 2-5 years preferred for advanced roles.
- Experience in hospice or home health is preferred.
- Ability to work safely and maintain alertness at all times.
Additional Information:
This is a full-time, benefit-eligible role with community travel within the Greater St. Louis Area. Prior experience in admissions or hospice is advantageous. The position offers a set schedule and mileage reimbursement, with potential bonuses for eligible candidates.
