Gilchrist Hospice Care seeks a Hospice Admissions Nurse to assess patients at home or in hospitals and prepare them for the spectrum of hospice services. The role also represents Gilchrist Hospice Care in the service area and collaborates with the Admissions team.
The nurse will perform initial assessments, create care plans with Medical Director approval, and educate patients and families while coordinating with social services and other departments.
