Hospice Admissions Navigator: Patient-Centered Home Care

Jobtailor is seeking a Nurse Navigator for our outpatient hospice program. You will be the first clinical contact for patients and their families beginning hospice services, perform assessments, coordinate care, and support patients at home or in care facilities.

You will collaborate with the Interdisciplinary Team and referral sources to organize admissions, document eligibility, and educate caregivers on end-of-life care and symptom management. Expect extensive driving as part of the role.
