Hospice Admissions Navigator

Lakeview Hospice is seeking an Admission Nurse to evaluate potential patients and families, obtain consents, and perform initial assessments to develop a plan of care for admission. The role involves coordination with the interdisciplinary group to ensure a smooth intake and alignment with the patient’s needs and goals.

You will join the IDG, coordinate admission activities, document findings, arrange medications and supplies, and provide teaching to patients and caregivers while adhering to
