Hospice Access Physician: Admissions & Care Lead

Chapters Health System is seeking a Hospice Access Physician to ensure physician services for the Patient Access Department, facilitating timely hospice care for referred patients. The role provides physician direction and education to admissions staff and completes regulatory hospice documentation for each admission.

Qualified candidates will hold an MD or DO, be licensed in Florida or eligible for required states, possess a DEA number, and hold a current BLS certification.
