Home Health RN Job Details

• Weekly Estimated Pay: $1,900-$2,000
• Location: Indianapolis, Indiana
• Healthcare Setting: Home health
• Schedule: Monday-Friday, day shift
• Shift Duration: 8-hour shifts
• Hours: Guaranteed 40 hours per week
• Contract Duration: 13-week contract
• Start Date: October 12, 2026
• End Date: January 10, 2027

Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license eligible for practice in Indiana
• At least 12 months of home health nursing experience within the past 3 years
• Previous OASIS experience required
• Experience with HCHB required
• Ability to complete comprehensive patient assessments and accurate clinical documentation
• Ability to meet a minimum productivity expectation of 30 points per week
• Strong communication, organization, and independent decision-making skills
• Reliable transportation and ability to travel throughout an assigned home health service area

Home Health Nursing Responsibilities

• Provide skilled nursing care to patients in their homes throughout the Indianapolis area
• Perform comprehensive patient assessments and develop individualized plans of care
• Complete OASIS assessments accurately and within required timeframes
• Monitor patient progress, symptoms, and response to treatment
• Administer medications and treatments in accordance with provider orders and established standards
• Educate patients and caregivers on disease management, medications, safety, and self-care
• Coordinate care with physicians, interdisciplinary team members, patients, and families
• Maintain timely and thorough clinical documentation in HCHB
• Identify changes in patient condition and communicate concerns promptly
• Deliver compassionate, patient-centered care that promotes safety, independence, and quality of life
