Home Infusion Clinical Liaison, RN Responsibilities include :

n
    n
  • n

    Facilitates transitions of care from acute and subacute settings to home infusion and education of home infusion and enteral therapies to hospital and community providers.

    n
  • n
  • n

    Assigned to selected facilities to collaborate with providers, case managers, referral sources, and home health agencies to facilitate patient discharge and coordination of care.

  • n

    Collaborates with business development team in gaining and maintaining market share through referral intake process.

    n
  • n
  • n

    Responsible for timely communication with all referral sources telephonically, through electronic platforms, and in-person.

    n
  • n
  • n

    Responsible for clear concise referral provided to pharmacy meeting all regulatory, payer, and safety requirements.

  • n
  • n

    Completes preadmission assessment and education to patient and caregiver of pharmacy services.

    n
  • n
  • n

    Conducts the initial infusion and enteral set-up and patient/caregiver training on our equipment and therapy services.

    n
  • n
  • n

    Initiates formulary changes and recommendations on route of feeding and advances in nutrition via the direction of the pharmacy's registered dietitian.

  • n

    Responsible for knowledge of Medicare and Managed Care regulations and requirements.

    n
  • n
  • n

    Reviews the patient's medical record to obtain both pertinent medical history and primary/secondary insurance information and communicates this to pharmacy intake.

  • n

    Participates with any data collection required for therapy start and patient tracking processes, which includes facilitating the transfer of orders via phone, fax, and e-prescribing.

  • n

    Timely response to referrals sources, providers, and leadership is essential.

    n
  • n
  • n

    Monitors and communicates referral source activity acting as one point of contact for referral sources, home health, and infusion.

The Home Infusion Clinical Liaison, RN Requirements:

    n
  • n

    BLS (Basic Life Support)

    n
  • n
  • n

    Driver's License - State of Florida

    n
  • n
  • n

    RN (Registered Nurse) License - State of Florida

    n
  • n
  • n

    Required Associate's or Diploma Nursing

    n
  • n
  • n

    Required 2 years of Nursing; And - 1 year of Infusion Related

    n
  • n
  • n

    Required Business Development skills

    n
  • n
  • n

    Requires Reliable transportation- Must be able to travel in assigned territory and attend meetings as needed.

    n
  • n
  • n

    Requires infusion and enteral administration skills assessment

    n
  • n
  • n

    Preferred Computer proficiency in Microsoft Office and Excel

    n
  • n
  • n

    Preferred experience in Home Healthcare specialty Pharmacy

    n
  • n
  • n

    Preferred CRNI Certification

    n
  • n
n
