About the Role: As a Case Manager in home health care, you will coordinate services with other healthcare professionals, communicate with insurance providers, and oversee visiting nurses. Your responsibilities extend to crafting individualized Plans of Care (POC) based on patient needs and family input, supporting treatment plan adherence, and monitoring overall well-being, medical requirements, and specialist treatments. Additionally, you'll recommend necessary community services. Your duties also encompass collaborating with the primary doctor to implement and adjust the plan, ensuring thorough documentation of all required information.

Job Responsibilities:

Coordinating home healthcare services in office.

Collaborating with healthcare providers, physicians, and different insurances.

Developing and implementing individualized Plans of Care (POC) based on patient needs and family feedback.

Supporting adherence to treatment plans and monitoring overall patient wellness.

Recommending and coordinating community services as necessary.

Collaborating with the primary doctor to ensure seamless plan implementation.

Making adjustments to the Plan of Care as required.

Completing all necessary documentation accurately and timely from the office.

Qualifications:

Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).

Minimum of 1+ years of experience in case management within a home healthcare setting.

Strong interpersonal and communication skills.

Ability to coordinate and collaborate with a multidisciplinary healthcare team.

Knowledge of regulatory requirements and best practices in home healthcare.
