Job Summary

Join a dedicated healthcare team as a Home Health Speech Therapist (SLP), providing compassionate, patient‑centered care in the comfort of patients' homes. In this role, you will evaluate, diagnose, and treat communication, cognitive, swallowing, and speech disorders while developing individualized treatment plans that promote independence and improve quality of life. Working collaboratively with physicians, nurses, therapists, patients, and caregivers, you will deliver skilled therapy services, monitor patient progress, and provide education to support successful outcomes. This position offers the opportunity to make a meaningful impact through personalized care while enjoying a high level of autonomy and flexibility in a home health setting.

Requirements

  • Undergraduate degree.
  • 1‑2 years of experience in Speech Therapy.
  • BLS (Basic Life Support) certification accredited by American Heart Association or American Red Cross.
  • Valid Texas driver’s license and proof of driver’s insurance.
  • Current licensure as a Speech Therapist.

Preferred

  • Experience in home health setting.

Job Duties

  • Evaluate patient’s home environment and communicative status.
  • Document identified problems, interventions, communications, and patient responses; ensure compliance with agency policies.
  • Develop and update Plan of Care (POC/POT) in coordination with physician when required.
  • Assess and record outcomes; make recommendations for improvement.
  • Provide patient care and coordinate with other disciplines and team members with assistance of the Clinical Therapy Manager.
  • Prioritize patient care as ordered by the attending physician and evaluate on an ongoing basis.
  • Instruct patients, family/caregiver, and health team members in methods to improve and manage their disability.
  • Adhere to speech pathology policies, regulations, codes of ethics, and federal, state, and local laws.
  • Follow infection control, hazardous waste, and HIV/HBV procedures per agency policy.
  • Participate in quality improvement activities and submit progress reports every 60 days and at discharge.
  • Maintain a safe and healthy environment; instruct patients on home safety.
  • Keep PPD and CPR up to date per agency policy.
  • Perform other duties as assigned.
