Registered Nurse (RN) - Home Health

Full-Time | 40 Hours per Week

Position Summary

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The RN will provide skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care while promoting independence, safety, and optimal health outcomes.

Essential Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing services in accordance with physician orders and agency policies.
• Administer medications, treatments, and wound care as prescribed.
• Monitor patient condition and document changes in health status.
• Educate patients, families, and caregivers regarding disease management, medications, and home care needs.
• Coordinate care with physicians, therapists, social workers, and other interdisciplinary team members.
• Maintain accurate and timely clinical documentation in the electronic medical record.
• Assess patient safety needs and recommend appropriate interventions and resources.
• Participate in case conferences and care coordination meetings.
• Ensure compliance with all federal, state, and agency regulations and standards.
• Respond appropriately to patient concerns and provide exceptional customer service.

Qualifications

• Active Registered Nurse (RN) license in the state of practice.
• Graduate of an accredited nursing program.
• Current BLS/CPR certification.
• Home Health experience preferred but not required.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible schedule.
• Valid driver's license, reliable transportation, and proof of auto insurance.
