Registered Nurse (RN) - Home Health Case Manager
Location: Columbia, SC (Richland County Coverage Area)
Position Type: Full-Time
Weekly Hour Guarantee: 40 Hours/Week
Overview: We are seeking a compassionate and experienced Registered Nurse (RN) Home Health Case Manager to provide high-quality, patient-centered care throughout Richland County, South Carolina . This role offers autonomy in scheduling, opportunities to build meaningful patient relationships, and the ability to make a lasting impact by helping patients remain safely in their homes.
Schedule:
- Full-time, 40 hours per week
- Agency office hours: Monday through Friday, 8:00 AM to 4:30 PM
Key Responsibilities:
- Complete admissions and start-of-care assessments for home health patients
- Develop, implement, and manage individualized care plans
- Provide ongoing case management for assigned patients
- Conduct comprehensive nursing assessments and reassessments
- Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members
- Provide clinical oversight and guidance to Licensed Practical Nurses (LPNs)
- Ensure accurate and timely documentation in compliance with regulatory and agency requirements
- Complete OASIS assessments and maintain quality standards
- Educate patients and families regarding diagnoses, treatment plans, medications, and disease management
- Assess, monitor, and guide wound care treatment as needed
- Monitor patient progress and adjust care plans accordingly
Required Qualifications:
- Active and unrestricted Registered Nurse (RN) license
- Current CPR Certification
- Home Health experience
