Home Health Registered Nurse (RN)

• Location: San Antonio, TX and surrounding areas


• Schedule: Monday-Friday, 8:00 AM-4:30 PM


• Weekly Hours Guaranteed: 36 Hours


• Coverage Area: Up to a 60-mile radius surrounding San Antonio, TX


• On-Call/Weekend Requirement: Yes, rotating on-call and weekend schedule


• Mileage Reimbursement: Yes, mileage reimbursed between patient visits

Position Overview

We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our growing team in the San Antonio, TX area. In this role, you will provide high-quality skilled nursing care to patients in their homes, helping them achieve optimal health outcomes while maintaining independence and comfort in their home environment.

The ideal candidate is a licensed RN with strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care.

Key Responsibilities

• Provide skilled nursing care to patients in a home health setting.


• Conduct comprehensive patient assessments and develop individualized care plans.


• Monitor patient conditions and document clinical findings accurately and timely.


• Administer medications, treatments, and other physician-ordered interventions.


• Educate patients and caregivers on disease management, medication compliance, and health maintenance.


• Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care.


• Ensure compliance with home health regulations, policies, and documentation standards.

Required Qualifications

• Active and unrestricted Registered Nurse (RN) license.


• Graduate of an accredited nursing program.


• Strong clinical assessment and critical-thinking skills.


• Valid driver's license and reliable transportation.


• Ability to travel throughout the assigned coverage area.


• Previous Home Health experience.


• Experience with Homecare Homebase (HCHB) software.
