Job Overview
We are seeking an experienced Home Health Registered Nurse (RN) for a 13-week assignment in the Fairfax, VA area. This position offers a consistent Monday–Friday schedule with no on-call requirements, making it ideal for nurses seeking work-life balance while delivering high-quality patient care in a home setting.
Responsibilities
- Provide skilled nursing care to patients in their homes
- Perform patient assessments and develop individualized care plans
- Complete accurate and timely documentation using HCHB (Homecare Homebase)
- Conduct OASIS assessments and ensure compliance with regulatory standards
- Educate patients and families on disease management and care plans
- Collaborate with physicians, therapists, and interdisciplinary team members
- Monitor patient progress and adjust care plans as needed
Territory
Coverage area includes Fairfax County, Prince William County, and surrounding areas within a 50-mile radius
Requirements
Active Registered Nurse (RN) license in Virginia
Minimum 1 year of home health experience within the last 3 years
HCHB (Homecare Homebase) experience required
OASIS experience required
Current BLS certification (AHA preferred)
Valid driver’s license and reliable transportation
