Title: RN - Home Health/Hospice

Duration: 3 months contract + Possibility of Extension

Location: Corpus Christi, TX

Coverage Areas: 60 mile radius from office

Shift: M-F 8 AM - 6 PM

back up on call one evening/night during the week and a weekend back up on call every 4-6 weeks

Roles & Responsibilities

• Manage a caseload of hospice patients across home and facility-based settings.
• Develop, implement, monitor, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Coordinate services to ensure continuity of care and timely response to patient needs.
• Conduct comprehensive nursing assessments and ongoing patient evaluations.
• Provide emotional support and compassionate guidance to patients and families.
• Advocate for patient goals, preferences, and care decisions.

Educate Patients And Caregivers Regarding

• Hospice philosophy and services
• Disease progression
• Symptom management
• Medication administration
• End-of-life care planning

Minimum Qualifications

• Active and unrestricted Registered Nurse (RN) License
• Valid driver's license and reliable transportation
• Current automobile insurance
• Ability to travel throughout the assigned service territory

Preferred Qualifications

• Previous Hospice Nursing Experience
• Previous RN Case Management Experience
• Travel nursing experience
• Experience providing care in homes, assisted living facilities, and skilled nursing facilities
• Strong knowledge of hospice regulations, symptom management, and end-of-life care practices
