Responsibilities:
- Provide comprehensive nursing care to home health patients in a designated geographic area.
- Perform patient assessments, wound care, and medication administration while managing chronic conditions such as CHF, COPD, CVA, and diabetes.
- Document treatments accurately using systems like EPIC, HCHB, OASIS, and Meditech.
- Educate patients and caregivers on health management and safety protocols.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
