We are hiring aRegistered Nurse (RN)for aHome Healthposition providing skilled nursing care to patients in their homes in the Westchester and surrounding areas. This position is eligible for a $15,000total bonus potential,including a$10,000first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional$5,000sign-on bonuspaid within the first18 monthsof employment!To receive these bonuses, you must be in good standing&meetperformance expectations, and in the role and status for which you were hired!

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
