Home Health Registered Nurse or Licensed Practical Nurse

About the Role

We are seeking a compassionate, dependable, and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide in‑home patient care near Hunters Point, AZ . The role involves delivering high‑quality nursing services in accordance with individualized plans of care.

Employment Type

This is a 1099 independent contractor opportunity with an average of 8 hours of nursing care per month, offering a flexible schedule.

Responsibilities

• Execute physician‑prescribed plans of care.
• Complete required documentation in the system of record.
• Administer prescribed medication, treatment, and therapies.
• Conduct patient assessments and coordinate care.
• Perform activities associated with daily living.
• Monitor vitals and GI intake and output.
• Assess skin integrity and administer wound care.
• Monitor for changes in the patient’s condition such as weight changes, self‑care abilities, and disease progression.
• Communicate with families, physicians, case managers, and other prescribed individuals regarding patient health.
• Provide education and training to the patient, the family, and the Home Health Aide.
• Work one‑on‑one in the patient’s home to create a comfortable environment.
• Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
• Provide other personal care services as necessary to meet the patient’s needs.

Minimum Qualifications

• Graduate of an accredited college or university; Associate or Baccalaureate School of Nursing.
• Current state or compact RN or LPN license.
• Current CPR certification.
• Valid TB test and physical examination.
• Required liability insurance.
• Good interpersonal skills.
• Hand‑eye coordination and manual dexterity.
• Ability to stand or walk for prolonged periods, lift up to 50 lbs, and move patients.
• Ability to operate under stress and make quick decisions.
• Reliable transportation and willingness to commute to patients’ homes.
• Ability to provide basic medical management up to medically fragile end‑of‑life care.

Preferred Qualifications

• At least 1 year of experience as an RN/LPN in a clinical or home‑health setting.
• Ability to assist with two‑person lifts.
