Aberdeen Assured Home Health, part of the Optum family, is seeking a Physical Therapist for home health. You will assess patient needs related to mobility and endurance, develop a treatment plan, perform interventions, and evaluate progress in a home setting.
Responsibilities include evaluating functional status, coordinating with the care team, documenting findings, and educating patients and families about the plan of care. Licensure and CPR are required.
