CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes. Reporting to the Clinical Manager, you will plan and administer prescribed physical therapy to achieve the highest level of function and quality of life.
You will assess patient strengths, document progress, develop rehabilitation plans, supervise PTAs and Home Health Aides, and coordinate with the care team to ensure timely evaluations and discharge
