Job Responsibilities
- Assess patient’s home environment for safety hazards and architectural barriers; identify equipment needs.
- Identify functional problems and establish goals and a plan of care designed to improve the patient’s functional status (revising as necessary).
- Perform patient tests, measurements, and evaluations such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement; record and evaluate findings to aid in establishing or revising specifics of treatment programs.
- Perform and complete start‑of‑care, and ongoing assessments and associated paperwork in accordance with agency policy.
- Periodically re‑evaluate and upgrade the patient’s program to maximize functional improvement.
- Administer the exercise program utilizing specialized therapeutic exercise techniques and equipment.
- Plan and administer prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts.
- Recommend prosthetic/orthotic devices for patients or evaluate their devices and make minor adjustments or recommend modifications by the prosthetist/orthotist.
- Confere with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
- Perform and submit required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
- All patient visit notes must be turned in the next scheduled workday, not to exceed forty‑eight (48) hours.

Education and Experience
- Graduate of an accredited Physical Therapy program and licensed by the state of Arizona.
- Previous experience in a clinical setting is preferred, but motivated new graduates are welcome.
- Excellent communication and interpersonal skills.
- Ability to work both independently and collaboratively in a team setting.
- Strong commitment to patient‑centered care.
