Job Description:

Heralds Home Health is seeking a Physical Therapist to join our dedicated team. We provide home health services in areas including Ranchos Palos Verdes, Long Beach, Torrance, Los Alamitos, Lakewood, San Pedro, and others. Our focus is on compassionate care for senior adults and individuals in need.

Must Have: Home health experience

Schedule:

Monday to Friday, 8:00 am to 5:00 pm

Qualifications:

• Degree from a baccalaureate or Master's program in physical therapy approved by an accredited organization
• Licensed to practice as a physical therapist within the state, if applicable
• At least two years of relevant experience; community/home health experience preferred
• Good verbal and written communication and organizational skills
• Current CPR certification
• Valid driver's license with insured and well-maintained vehicle

Benefits:

• Salary: $55.00 to $60.00 per hour

Responsibilities:

• Provide physical therapy services according to a physician’s plan of care, including assessment, treatment, and evaluation of the patient's home environment
• Direct physical therapy treatments and instruct patients’ families or caregivers
• Participate in case conferences and develop individualized care plans
• Offer consultation and in-service education to home health staff as needed
