St. David’s Medical Center seeks a Home Health Physical Therapist to provide one-on-one, evidence-based rehab services to patients in their North Austin homes. You will perform comprehensive evaluations, develop individualized treatment plans, deliver therapeutic interventions, and educate patients and caregivers to improve mobility, safety, and independence.
Collaborating closely with a multidisciplinary team, you’ll document in EMRs, manage a field-based caseload, and contribute to our patient-centered culture focused on clinical excellence and compassionate care.
