Home Health Visits with Growth Potential

Responsibilities

• Provide physical therapy assessments, interventions, and care planning for home health patients.
• Perform admissions, evaluations, routine visits, and discharges per Medicare and Medicaid guidelines.
• Educate patients and caregivers on therapy plans, techniques, and home exercise programs.
• Document patient progress accurately and in a timely manner.
• Collaborate with interdisciplinary teams to ensure quality care.

Growth Opportunities

• Remote Therapeutic Monitoring (RTM): Utilize RTM technology to monitor and manage patient progress remotely. Ensure compliance with RTM CPT codes and Medicare billing requirements.
• Outpatient Physical Therapy Services: Provide therapy services in a clinic setting or through telehealth. Deliver personalized treatment plans for musculoskeletal and other conditions.

Requirements

• Willingness to obtain a Medicare Provider ID.
• Services will be billed under a group using the group’s Provider ID.
