Renew Home Health is looking for a compassionate Physical Therapist (PT) to help patients regain strength and independence in their homes across the Granbury area. In this PRN role, you’ll evaluate patients, build individualized therapy plans, and guide their recovery where they’re most comfortable — at home. What you’ll do as a Physical Therapist

• Evaluate patients and develop individualized therapy plans of care
• Provide skilled physical therapy visits in patients’ homes throughout the Granbury area
• Help patients improve strength, mobility, balance, and independence
• Adjust the plan of care as patients progress and collaborate with the care team
• Teach patients and families home exercises and safety strategies
• Document each visit accurately using our AI-assisted charting tools

What we’re looking for

• Active Texas Physical Therapist (PT) license in good standing
• Home health experience a plus
• A valid Texas driver’s license, reliable transportation, and current auto insurance
• Strong assessment skills and sound clinical judgment
• A caring, patient-first attitude
