We are hiring aPhysical Therapist (PT)for aHome Healthposition providing skilled care to patients in their homes in the Petoskey and surroundingareas and is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus.

WhatYou’llDo:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough,compliantEMRdocumentation
• Work independently,but never alone
• Travel to patient homes within assigned territory

PTQualifications

• ActivePhysical Therapist, PTlicense in the state you work
• One year of experienceas a Physical Therapistandthe clinicalexpertiseto deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
