Position Summary

As a Home Health Physical Therapist, you will provide skilled rehabilitation services to patients in their homes, helping them improve mobility, restore function, and achieve their highest level of independence. You'll collaborate with an interdisciplinary care team while developing individualized treatment plans tailored to each patient's unique needs and goals.

Key Responsibilities

• Conduct comprehensive in-home physical therapy evaluations and assessments


• Develop, implement, and update individualized treatment plans


• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence


• Monitor patient progress and modify treatment plans as appropriate


• Educate patients and caregivers on exercises, safety techniques, mobility strategies, and injury prevention


• Accurately document visits and patient outcomes in accordance with agency standards and regulatory requirements


• Communicate patient progress and care recommendations with physicians and the interdisciplinary healthcare team


• Maintain compliance with Medicare, state, agency, and professional practice standards

Qualifications

Required:

• Current Illinois Physical Therapist (PT) license in good standing


• Doctor of Physical Therapy (DPT) degree


• Valid driver's license and reliable transportation


• Strong communication, organization, and documentation skills


• Ability to work independently while managing a flexible caseload


• Professional, dependable, and patient-focused approach to care

Preferred

• Previous home health experience


• Knowledge of Medicare home health regulations and documentation standards
