Canyon Home Health seeks a compassionate Physical Therapist to deliver home-based care in Ogden/South Ogden, UT. You’ll manage a flexible caseload with autonomy, visiting patients in their homes and coordinating with physicians to optimize progress.
You’ll supervise PTAs and HHAs as needed, maintain up-to-date documentation (OASIS, progress notes, discharge summaries), and apply sound clinical judgment at every visit to help patients recover and stay independent.
