St. David’s Medical Center is seeking a Home Health Physical Therapist to deliver high-quality, evidence-based rehabilitation services to patients in their homes across North Austin. In this field-based role, you will perform comprehensive assessments, develop individualized treatment plans, and provide gait, balance, and mobility training to support recovery and independence.
You’ll collaborate closely with physicians, nurses, and other clinicians, documenting care in our EHR, educating patients and families, and promoting safety and fall prevention.
