Job Description

As a Physical Therapist, you will:

• Provide one-on-one physical therapy services in patients' homes
• Assess and develop individualized treatment plans
• Monitor patient progress and modify plans as needed
• Collaborate with interdisciplinary teams when required

Qualifications

• Doctorate or Degree in Physical Therapy (DPT or equivalent)
• Minimum 2 years of experience in physical therapy (Home Health experience preferred)
• Strong communication and organizational skills
• Self-motivated with the ability to work independently and manage a flexible caseload
• Valid California Physical Therapist License
• Valid California Driver's License
• Current CPR certification, TB Test, and Professional Liability Insurance
• Pre-employment physical and TB test (within the last 6 months)

Additional Information

Pay: $62.50 - $120.00 per hour
