Artemis Healthcare Staffing is seeking a dedicated and passionate professional to join our client's team!

Position Name: Home Health Hospice Registered Nurse (RN)

Location: Santa Cruz, CA or Oceanside, CA

Setting: Home Health & Hospice

Pay

- Paid Per Visit Range: $60-$237.50 (depends on years of experience and location).

- RNs are paid per visit using a point-based system. Each visit type is assigned a point value, and your pay is based on your Point Rate x Number of Points you complete in a week.

Shift

- Full-Time, Normal Business Hours

Qualifications

Required:

- At least 1+ year of Home Health Experience

- RN License

- Experience with Adult patient population

- CPR Certified

- Current and valid Driver's License and Auto Insurance

Preferred:

- Homecare Homebased EMR experience

- Several years experience as a Home Health RN

Job Summary

- Conduct initial and ongoing comprehensive assessments of patients and families to identify hospice care needs.

- Deliver professional nursing care by applying the nursing process in accordance with state regulations.

- Create and regularly update a patient-specific plan of care based on assessments and nursing diagnoses.

- Provide counseling and health education to patients and families, facilitating their involvement in care decisions.

- Help patients with activities of daily living, promoting self-sufficiency and comfort.

- Maintain accurate and up-to-date electronic medical records (EMR) in line with organizational policies.

- Act as a coordinator for patient care, ensuring effective communication with physicians and community health resources.

- Work closely with interdisciplinary teams to address the emotional and physical needs of patients and families.

- Supervise support staff as needed and engage in continuous professional development through education.

- Take part in quality assessment and performance improvement initiatives to enhance patient care services.
