Key responsibilities:

Skilled Nursing Care: Administering medications, injections, wound care, catheter care, and tube feedings.

Patient Monitoring: Taking vital signs, observing physical/mental status, and reporting changes to the supervising RN.

Patient & Family Education: Teaching self-care techniques, medication management, and disease processes.

Documentation: Maintaining accurate records of observations, interventions, progress, and communications.

Care Coordination: Reporting patient needs to the RN and other team members (PT, OT, Social Work).

Supportive Care: Assisting with daily activities like bathing, dressing, and providing companionship.

Patient-focused: Their primary role is patient interaction and management.

Hands-on care: They work directly with patients to alleviate suffering and improve health.

Specialized Skills & Requirements:

IV Certification: Many home health agencies require LVNs to be IV-certified to initiate and maintain peripheral intravenous therapy.

Autonomy: Ability to work independently in diverse home environments while managing a traveling schedule, typically within a 5–15 mile radius.

Technology Proficiency: Experience with electronic medical record (EMR) systems (Wellsky, Kinnser) is standard for mobile documentation.

Licensure & Ethics: Must hold a current, unrestricted state LVN license and maintain a valid driver’s license with reliable transportation and insurance.

Safety Awareness: Inspecting home environments for hazards to ensure patient and clinician safety during visits.

Essential Skills & Environment:

Autonomy & Travel: Work independently, traveling between homes, often requiring schedule flexibility.

Collaboration: Strong communication skills to work within an interdisciplinary team.

Problem-Solving: Differentiate urgent from non-urgent situations and manage varied patient needs.

Technical Skills: Proficiency with required technology for documentation and communication. Works independently in various patient homes, often managing multiple patients.

Requires strong time management, problem-solving, and independent decision-making within their scope.

Adapts care plans based on the unique resources and challenges of the home setting.
